Care Coordinators

Primary Digital Health Care coordinators are experts at facilitating appointments with our providers for members who wish to be seen for a primary care visit. Once a primary care visit is requested, the care coordinator will schedule the member at the next available appointment time with one of our providers who participates in the primary care program. The provider will communicate with the care coordinator before, during, and after the visit to facilitate ongoing care management.

Virtual Primary Health (VPC) - New Member Visit, How to get started.

During the new member visit, the Primary Digital Health primary care provider will spend up to 45 minutes with the member to understand their full medical history and background to provide them with the best care possible. This will involve taking a comprehensive history including social and family history, prior screening and immunization status, a virtual physical exam, and development of an ongoing plan for the initial and future needs of the member. During this visit, the provider will order any labs or diagnostics that are needed based on the needs of the member as well as order any immunizations or medications that the member may need. In addition to addressing any acute complaints, the provider will also encourage long-term wellness activities and create a future plan with the member.

If the member already has a primary care provider (PCP), they can still use our service to fill any gaps in their care including co-management of chronic disease, treating acute illness due to long-wait times with their PCP, or facilitating the coordination of screening tests and immunizations. If the member does not have a local PCP, our provider will become the member's PCP and follow this member begin a long-standing relationship and develop a whole-person approach to their health.

• Acute Drugs can be pick up at Walmart and over 65,000 pharmacies.
• Maintenance Drugs are a mail order program with Welldyne live customer service or online ordering.
• Insulin Savings are offered exclusively at the 4,000+ Walmart locations.
• The Retail Rx Saving Card is accepted at 65,000 national retail and independent pharmacies.

Mental Health Benefit Access to Counseling

Members receive the support they need by adding our virtual mental health tools to your benefits. We provide a safe, secure, and private means of seeking mental health assistance from licensed mental health professionals via virtual or telephonic counseling sessions.

How does the Vision Service Plan work?

• WellVision Exam^®: $50 with the purchase of a complete pair of prescription glasses (once every calendar year) or 20% savings without a purchase.

How does MPC Dental Savings Program Work?

My Primary Care Dental Savings Program - Utilizing the Aetna Dental Access^® Network

Access a national network of over 262,000 dental practice locations. Participating dental locations provide savings that may range from 15-50%* per visit, on average, on most dental services including cleanings, x-rays, fillings, root canals, crowns, bridges, and orthodontia.
